临床误诊误治

目的 分析超声引导下一次性使用动静脉留置针改良法肾囊肿穿刺硬化术治疗肾囊肿的效果。方法 选取2019年1月至2023年9月收治的98例肾囊肿，根据治疗方案分为观察组48例和对照组50例，观察组在超声引导下行一次性使用动静脉留置针改良法肾囊肿穿刺硬化术，对照组在超声引导下行常规肾囊肿穿刺硬化术。比较两组临床效果、围术期指标、术后并发症、术后复发率及手术前后囊肿体积、腰腹疼痛视觉模拟评分法（VAS）评分。结果 观察组治愈率高于对照组，术后复发率低于对照组（P<0.05）。观察组手术时间及术后离院时间短于对照组，无水乙醇灌洗次数及治疗时痛感发生率均低于对照组（P<0.01）。两组术后即刻、1个月、3个月囊肿体积、腰腹疼痛VAS评分均较术前减小（P<0.05）；观察组术后即刻、1个月、3个月囊肿体积、腰腹疼痛VAS评分小于对照组（P<0.05）。观察组术中出血发生率低于对照组（P<0.05）。结论 超声引导下一次性使用动静脉留置针改良法肾囊肿穿刺硬化术治疗肾囊肿具有良好的安全性和可靠性，可缓解术后腰腹疼痛，减少术中出血，提高治愈率，降低术后复发率。

Objective To analyze the effect of ultrasound-guided modified renal cyst puncture sclerotherapy using disposable arteriovenous indwelling needles in the treatment of renal cysts. Methods A total of 98 patients with renal cysts admitted from January 2019 to September 2023 were selected and divided into the observation group(n=48) and the control group(n=50) according to the treatment plan. The observation group was treated with ultrasound-guided modified renal cyst puncture sclerotherapy using disposable arteriovenous indwelling needles for renal cysts, while the control group was treated with ultrasoundguided conventional renal cyst puncture sclerotherapy. The clinical effect, perioperative indexes, postoperative complications and postoperative recurrence rate were compared between the two groups, as well as cyst volume and visual analogue scale(VAS) score of lumbar and abdominal pain before and after operation. Results The cure rate of the observation group was higher than that of the control group, while the postoperative recurrence rate was lower than that of the control group(P<0.05). The duration of operation and postoperative discharge time in the observation group were shorter than those in the control group, and the times of absolute ethanol lavage and the incidence of pain during treatment were lower than those in the control group(P<0.01). The cyst volume and VAS scores of lumbar and abdominal pain in the two groups immediately after operation, and at 1 month and 3 months after operation were smaller than those before operation(P<0.05). The cyst volume and VAS score of lumbar and abdominal pain in the observation group were smaller than those in the control group immediately after operation, and at 1 month and 3 months after operation(P<0.05). The incidence of intraoperative bleeding in the observation group was lower than that in the control group(P<0.05). Conclusions Ultrasound-guided modified renal cyst puncture sclerotherapy using disposable arteriovenous indwelling needles for renal cysts has good safety and reliability, which can relieve postoperative lumbar and abdominal pain, reduce intraoperative bleeding, improve cure rate and reduce postoperative recurrence rate.
