桥本甲状腺炎患者外周血miR-27a、IL-27水平及临床意义

目的 探讨桥本甲状腺炎(HT)患者外周血微小核糖核酸-27a(mi R-27a)、白细胞介素-27(IL-27)水平及临床意义。方法 选取2022年1月至2024年2月收治的120例HT患者作为HT组,另选同期体检的健康者100例作为对照组。收集两组一般资料及甲状腺功能参数,检测并比较两组外周血mi R-27a、IL-27。采用Pearson相关分析mi R-27a、IL-27与甲状腺功能参数的关系,采用受试者工作特征(ROC)曲线分析mi R-27a、IL-27水平对HT的诊断价值。结果 HT组甲状腺疾病家族史占比、促甲状腺激素(TSH)、甲状腺球蛋白抗体(Tg Ab)、甲状腺过氧化物酶抗体(TPOAb)、mi R-27a、IL-27明显高于对照组,游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)低于对照组(P<0.05,P<0.01)。miR-27a、IL-27水平与HT患者TSH、TgAb、TPOAb呈正相关(P<0.05,P<0.01),与HT患者FT3、FT4无明显相关性(P>0.05)。多因素logistic回归分析显示,有甲状腺疾病家族史、高水平Tg Ab、高水平TPOAb、高水平mi R-27a、高水平IL-27是导致HT发生的危险因素(P<0.05,P<0.01)。ROC曲线分析显示,mi R-27a、IL-27联合检测诊断HT的曲线下面积显著高于单独诊断(P<0.01)。结论 HT患者外周血mi R-27a、IL-27水平升高,且与患者甲状腺功能和抗体有关,mi R-27a、IL-27水平升高增加HT发生风险,对HT的早期诊断具有一定价值。

Abstract:

Objective To investigate the expression and clinical significance of microrNA-27a(miR-27a) and interleukin-27(IL-27) in peripheral blood of patients with Hashimoto's thyroiditis(HT). Methods A total of 120 patients with HT admitted from January 2022 to February 2024 were selected as the HT group, and 100 healthy people who underwent physical examination during the same period were selected as the control group. The general data and thyroid function parameters of the two groups were collected, and miR-27a and IL-27 in peripheral blood were detected and compared between the two groups. Pearson correlation analysis was used to analyze the relationship between miR-27a, IL-27 and thyroid function parameters. Receiver operating characteristic(ROC) curve was used to analyze the diagnostic value of miR-27a and IL-27 levels in HT. Results The proportion of family history of thyroid disease, thyroid stimulating hormone(TSH), thyroglobulin antibody(TgAb), thyroid peroxidase antibody(TPOAb), miR-27a and IL-27 in the HT group were significantly higher than those in the control group.Free triiodothyronine(FT3) and free thyroxine(FT4) were lower than those in the control group(P<0.05 or P<0.01). The levels of miR-27a and IL-27 were positively correlated with TSH, TgAb and TPOAb in patients with HT(P<0.05 or P<0.01), but were not significantly correlated with FT3 and FT4 in patients with HT(P>0.05). Multivariate logistic regression analysis showed that family history of thyroid disease, high level of TgAb, high level of TPOAb, high level of miR-27a and high level of IL-27 were risk factors for HT(P<0.05 or P<0.01). ROC curve analysis showed that the area under the ROC curve of combined detection of miR-27a and IL-27 was significantly higher than that of single diagnosis(P<0.01). Conclusions The levels of miR-27a and IL-27in peripheral blood of patients with HT are highly expressed, and are related to thyroid function and antibodies. The elevated levels of miR-27a and IL-27 increase the risk of HT, and have certain value in the early diagnosis of HT.
